Methods: Among male patients with NMIBCa who underwent transurethral resection of bladder tumor (TUR-B) between January, 2009 and April, 2017, we evaluated the intravesical recurrence rate of bladder tumor within the patients receiving 5a-reductase inhibitor (5-ARI) therapy (finasteride or dutasteride) for benign prostatic hyperplasia. Results: Total 162 males, 26 patients with finasteride, 35 patients with dutasteride and 101 patients without 5-ARI therapy, were studied. The overall median follow-up was 3.8 years, and a intravesical recurrence developed in 12 (46.2%), 10(28.6%) and 54 (53.5%) patients in finasteride, dutasteride and no 5-ARI groups, respectively. On Cox regression analysis, multiple tumors (HR 1.21, 95% CI 1.02-1.91, p =0.046), intravesical therapy with mitomycin + BCG (HR 0.79, 95% CI 0.85-3.07, p =0.039) and androgen suppression therapy with dutasteride (HR 0.27, 95% CI 0.12-0.89, p =0.030) were significantly associated with intravesical recurrence. However, finasteride failed to achieve independent predictor status of intravesical recurrence. Furthermore, no patient among the dutasteride group showed a pathologic progression to muscle invasive bladder cancer (MIBCa). Conclusions: According to our study results, androgen suppression therapy with dutasteride could be used as a possible prophylactic treatment for intravesical recurrence of BCa who underwent TUR-B due to NMIBCa. Dutasteride also prevented male NMIBCa patients from progression to MIBCa. However, further researches are still warranted to confirm the effect of dutasteride on NMIBCa. keywords : bladder neoplasm, neoplasm recurrence, dutasteride
