Long-term experience of sperm cryopreservation in cancer patients in a single fertility center

Purpose: Sperm cryopreservation before cancer treatment is the most effective method to preserve male fertility. We present our 21years long-term experience of sperm cryopreservation in cancer patients regarding semen quality as well as current status of cryopreserved sperm including sperm use rate.
Materials and Methods: A total of 721 patients with cancer performed sperm cryopreservation for fertility preservation from January 1996 to December 2016. Medical charts were reviewed on patient age, marital status, cancer type, semen volume, sperm counts and motility, length of storage and current banking status.
Results: The cancer diagnoses of the 721 patients were leukemia(28.4%), lymphoma(18.3%), testis cancer(10.0%). Mean age at cryopreservation was 27.0 years(13~65years). In 111(15.4%), sperm cryopreservation was done during or after cancer treatment. Mean sperm concentration was 66.7 ± 66.3 x106/mL and mean sperm motility was 33.8 ± 16.3%. During median follow-up duration of 75months(1~226months), 44(6.1%) used their banked sperm at this fertility center for ART and 9(1.3%) transferred their banked sperm to other center. Median duration from cryopreservation to use is 51months(1~158months).
Conclusions: Because sperm cryopreservation before gonadotoxic treatment is the most reliable method to preserve male fertility, sperm cryopreservation should be performed as a standard of care to all men who will have cancer therapy. Even though 6.1% used their banked sperm at this center for ART and 1.3% used at another center during follow-up duration, a consistent proportion of men who did not use their sample at the time of the analysis is expected to use it in the future.
