1 year results of a minimal invasive intra-prostastic implant; Urolift for benign prostatic hyperplasia treatment in Korea

Purposes:
As a minimal invasive device for benign prostatic hyperplasia (BPH) treatment, Urolift is widely accepted in the worldwide but it is not widely used in Korea. We investigated the 1 year results of urolift for patients with BPH in Korea.

Materials and methods:
From April 2017 to June 2018, 45 patients with BPH who were not satisfied with a blockers were treated with urolift under local anethesia with sedation. Foley urethral catheter was placed post operation and removed on the same day. Whenever possible, patients were scheduled to be discharged on the day, but someone was admitted to the hospital due to the patient's personal reason. IPSS and maximum urinary flow rate and post voiding residual urine were evaluated preoperatively and 1, 3, 6, 12 months later.

Results:
Mean age was 68.83 ±8.61 years old and mean prostatic volume was 37.0±12.28g. No severe adverse event was observed postoperatively. Preoperative total IPSS and QOL were 20.11±7.08 and 3.64±1.40. Total IPSS was significantly improved to 9.00±5.061 (p<0.001) and QOL was 2.48±1.31 (p=0.006) after 1 month respectively. And IPSS and QOL showed some worse after 3 months (11.88±4.64, 2.48±1.31) but remained well until 6 and 12 months compared to baseline.
And preoperative Qmax was 7.95±6.31 and 1 month after surgery, it was significantly improved to 12.85±6.99 (p<0.001). It was remained well after 3, 6, 12 months (12.41±7.28, 12.04±7.92, 12.50±7.40). However post voiding residual urine were not significant after 1 month (59.64±66.75 to 40.33±78.71 (p=0.73)) and so on. Mean hospital day was 1.71±0.48. No patient reported retrograde ejaculation.

Conclusions:
We evaluated 1 year efficacy of Urolift, a new minimally invasive device for BPH treatment in Korea, and showed significant improvement of IPSS, QOL and Qmax. Long term follow-up is needed, but it is expected that not only the improvement of voiding symptom but also the preservation of sexual function is possible.
