Risk Factors for Postoperative Systemic Inflammatory Response SyndromeAfter Percutaneous Nephrolithotomy

Objective:The objective of this studyis to evaluate the risk factors of infectious complications after percutaneous nephrolithotomy (PCNL) and build a prediction tool for postoperative complications based on the risk factors.
Methods:A total of 106 patientswho underwent PCNL for renal stones between 2014 and 2015 in our institute by asingle surgeon were included. A detailed clinical information and laboratory results were obtained from the electronic medical records. Systemic inflammatory response syndrome (SIRS) was checked after PCNL surgery, andSIRS was diagnosed whentwo or more of the following criteria were observed in the patients: (1) body temperature > 38°C or < 36°C; (2) heart rate > 90 bpm; (3) respiratory rate > 20 breaths/min or PaCO2 < 32mmHg; (4) white blood cell (WBC) count > 12,000 cells/µL or < 4,000 cells/µL.Pre- and perioperative variables that may have contributed to the development of SIRS after PCNL were included in the analysis. Univariable test was performed by using Student T-test and Chi-square test for continuous and categorical variables, respectively. Logistic regression test was performed as multivariable test which included the variables that were stataistically significant from the univariable tests.
Results:In all, 17cases (16.04%) developed SIRS after PCNL. Among the SIRS cases, fever > 38°C and serum WBC count > 12,000 cells/µL were observed in 4 (3.77%) and 14 (13.21%)cases, respectively. In univariable analysis, age (p = 0.03),stone size (p = 0.01), urine WBC (p = 0.02), Staghorn stone(p = 0.001) and total operation time (p<0.001) were related to the development of SIRS.In multivariate analysis, urine WBC (p<0.001; OR = 1.03, CI:1.01–1.05) and total operation time (p<0.001; OR = 1.02, CI:1.01–1.03) were related to the development of SIRS after PCNL.
Conclusions:Patients with higher urine WBC and longer operation time may have a higher risk of developing SIRS after PCNL.
