Infertility & Sexual Dysfunction(구연) Oral Session 2 / Infertility & Sexual Dysfunction (O-015) Crystal A (3F) 11월28일(수) 13:00-14:00 발기부전 환자에서 PDE-5 차단제의 치료반응 예측 수단으로서 맥파속도 측정의 의미 ¹가톨릭대학교 의정부성모병원 비뇨기과학 교실, ²건국대학교 충주병원 비뇨기과학 교실 최우석¹, 박지운¹, 정준세¹, 배상락¹, 박봉희¹, 이용석¹, 한창희¹, 강성학¹, 김홍섭², 정홍² Objective : The objectives of this study is to identify the predictive value of pulse wave velocity at treatment response in phosphodiesterase-5- inhibitor to erectile dysfunction in erectile dysfunction Material and Methods : From July 2014 to August 2017, the outpatients who visited outpatient department for complaining the erectile dysfunction were included. All patients had done IPSS, IIEF-5 questionnaire and checked physical examination, vital sign, BMI, liver profile, renal profile, serum PSA and testosterone level and pulse wave velocity(PWV) test. According to PWV results, patients were divided into two group; non-severe vessel stiffness group(non-severe group) and severe vessel stiffness group(severe group). All patients take PDE 5 inhibitor sildenafil for 4 weeks. After 4 week medication, patient also check serum profile and PWV, IPSS and IIEF5 again. Results : Total 34 patients was included in the study. Patient’s ages and BMI did not have difference between two groups. Serum liver and kidney profile, PSA and testosterone level also had no significant difference. IPSS did not show any significant difference between two group at each time and no significant change after medication. But in IIEF5, there was no significant increase in the number of erections(-0.7 vs. 0.5, p=0.029), satisfaction(0.75 vs. 1.14, p=0.001) and total score(-0.86 vs. 3.46, p=0.012) in the severe vessel stiffness group, but in the normal or mild vessel stiffness group had significant improvement after medication. Conclusions : Severe vessel stiffness by PWV was predicted of poor response on PDE 5 inhibitor treatment. There are some limitation because the number of subjects is small. Further study is needed for larger population for more precise predictive index for identifying response of PDE 5 inhibitor. keywords : Pulse Wave Velocity, PDE-5 inhibitor, Predictive value
