MRI/US fusion biopsy 시행시, PI-RADS 점수와 전립선암 검출률 간의 상관관계에 관한 연구

Aim : aim of this study was to analyse the correlation between PI-RADS Score and Prostate Cancer Detection Rate on MRI/US fusion-guided biopsy
Methods : 179men(age range 40 to 85) with clinical suspicion for PCA underwent multiparametric MRI of the prostate and suspicious lesions were found by radiologists and graded PI-RADS score. And then, targeted Biopsy was performed through MRI/US fusion-guiding.
Results : In total 179men, 27 patients showed below PI-RADS score 2, 69 patients showed PI-RADS score 3, 46 patients showed PI-RADS score 4 and 37 patients showed PI-RADS score 5. In each group, the rate of PCa. detection rate were 3.7%, 14.5%, 39.1% and 75.7%. There was linear-by-linear association between PI-RADS score and Cancer Detection. And there was significant difference in the detection rate of PCa between groups(p = 0.00).
The significant PCa.(GS ≥ 7) detection rates were 8.7% in PI-RADS score 3 and 19.6% in PI-RADS score 4. But, there was no significant difference of the Significant PCa detection rate between PI-RADS score 3 and 4
Conclusion : the PCa. detection rate on MR/US fusion-guided biopsy increases according to PI-RADS score grading. We need to perform MRI/US fusion-guided Target Biopsy on the PI-RADS score 3(equivocal) lesion because there was no significant difference of the Significant PCa. detection rate between PI-RADS score 3 and 4
