Introduction: We aimed to estimate the impacts of metabolic syndrome (MetS) and life-style factors on the risk of prostate cancer using large retrospective cohort derived from National Health Insurance Service health checkup cohort.
Materials and methods: A total number of 130,342 men included in the health checkup cohort at 2009 and followed-up until 2013, were divided into 2 groups according to the presence of prostate cancer, and study population was additionally divided according to age groups. A generalized estimating equation was used to assess the impacts of MetS / each component of MetS and life style on the prevalence of prostate cancer after adjusting other variables.
Results: Prostate cancer was detected in 2,369 men (1.8%) during follow-up. The prevalence of prostate cancer in the overall cohort with MetS was significantly higher than in those without MetS throughout the entire duration of the follow-up. Multivariable analysis showed that MetS was associated with the prevalence of prostate cancer (OR: 1.170, p = 0.002), in addition to year at evaluation (OR: 1.118, p < 0.001) and age (OR: 1.094, p < 0.001). Among the components of MetS, decreased HDL-cholesterolemia (OR: 1.303, p <0.001) was the strongest factor for the increased prevalence of prostate cancer after adjusting other variables. Central obesity was determined as another factor for the increased prevalence of prostate cancer among the components of MetS (OR: 1.149, p < 0.001).
Conclusions: MetS and its components, especially decreased HDL-cholesterolemia, and alcohol intake, are determined as a related factor for the development of prostate cancer. Preventing MetS, maintaining adequate HDL-cholesterol level, and preventing central obesity could be helpful for preventing prostate cancer.
