Aim: We investigated the relationship between pelvic floor muscle strength and erectile dysfunction in a prospective cohort of men.
Materials and Methods: Among the 270 consecutive men who visited the Urology outpatient clinic for complaints other than voiding/erectile dysfunction, we investigated 247. Patients with neurologic deficits, abnormal pelvic bones, history of pelvic radiation therapy, prostatectomy or anorectal surgery, urinary incontinence, or erectile dysfunction were excluded. Patients were instructed to use the AnyKegel device (Furun Medical, South Korea) and dynamometric measurements were recorded. We recorded the maximal strength (kilogram-force, kgf), mean strength (kgf), endurance, the International Index of Erectile Function-5 (IIEF-5) scores, and patient demographics.
Results: We analyzed 247 patients with a mean age of 62.8 (±10.1 [standard deviation]) years. Mean maximal strength was 2.0 (±1.5) kgf (range 0.1–9.5). Mean strength was 1.1 (±0.8) kgf (range 0.0–5.6). Mean endurance was 7.2 (±2.6) s (range 0.2–12.5). Mean IIEF-5 score was 13.3 (±7.9). Age, the International Prostate Symptom Score, and the IIEF-5 were not associated with pelvic muscle strength and endurance. Patients with IIEF-5 scores ≤12 tended to be older, with a higher occurrence of hypertension and a lower body mass index (BMI). Multivariate logistic regression analysis showed that age (odds ratio [OR] 1.08, 95% confidence interval [CI] 1.04–1.12, p<0.001), and maximal strength <1.9 kgf (OR 2.62, 95% CI 1.38–4.97, p=0.003) were independent predictors for IIEF-5 scores ≤12 when adjusted for prostate cancer, hypertension, BMI, and endurance.
Conclusions: Patients with erectile dysfunction tend to be older with lower maximal strength of the pelvic floor muscles, which might be enhanced by pelvic floor muscle exercises.
