We retrospectively evaluated the incidence of testicular atrophy (TA) after testicular salvage in TT and identified associated factors. Methods: We reviewed patients who underwent emergency operation suspicious for testicular torsion in our institution from January 2013 to March 2017. Sixty five boys with a median age at presentation of 7.8 (range 4-16) years underwent surgical treatment for TT. Data was collected on patient demographics, duration of pain, sonographic findings, torsion rotation, postoperative complications, and follow-up assessment of TA at 12 month postoperatively (defined as difference in testicular volume > 30% compared with the contralateral testis, based on measurement by ultrasound. The testis volume was calculated with the formula for an ellipsoid = /6 X length X width X height). Results: Of 65 patients who had scrotal exploration for testicular torsion, 31 had testicular salvage (orchiopexy, rate 47.69%) and 34 orchiectomy (orchiectomy rate 52.31%). Median follow-up was 12 months. All patients with preoperative no viability of testis underwent orchiectomy (n=28). Factors associated with TA were duration of pain ≥ 12hr (p<0.001) and degree of testis rotation (p=0.041). There was no significant difference in testicular volumes before (2.731 ± 1.241) and after surgery (2.143 ± 1.673) based on sonographic evidence of reduced vascularity of testis (p=0.783). Conclusions: We were able to derive factors associated with postoperative TA after TT surgery. Pain duration ≥12 hours and degree of rotation (≥360°) was associated with postoperative TA (p<0.001, p=0.041). Preoperative decreased testis vascularity on ultrasound was not a predictor of TA after salvage. keywords : Testicular torsion, salvage, testicular atrophy
