Introduction: Recently, nephron sparing surgery in upper tract urothelial carcinoma (UTUC) is becoming an issue. So, in this study, we compare oncologic and functional outcomes between radical nephroureterectomy (RNU) and segmental ureterectomy (SU) in UTUC.

Materials and Methods: We retrospectively analyzed records of 394 patients treated with RNU from 2008 to 2016 and 38 patients treated with SU from 2009 to February 2018. Progression free survival (PFS), cancer specific survival (CSS), overall survival (OS), upper tract cancer recurrence free survival, bladder cancer recurrence free survival and eGFR preservation rate after operation were examined.

Results: Patient characteristics are shown at Table 1. Difference of PFS (p = 0.469), CSS (p = 0.711), OS (p = 0.119) and bladder cancer recurrence free survival (p=0.924) between RNU and SU were not statistically significant. (Figure 1.) And, in the multivariate regression model for progression, SU is not associated with the progression after UTUC surgery. However, in upper tract cancer recurrence, SU shows more chance to recur than RNU. (98.1% vs 92.2% at 3-year follow up, p = 0.001) (Figuire 1.)
In terms of renal function, eGFR preservation rate after 1 year of operation, SU was superior to RNU. (78.3±24.6 % vs 109.2±29.8 %, p < 0.001)

Conclusion: RNU and SU showed no signficant difference in PFS, CSS, OS and bladder cancer recurrence. But upper tract recurrence is more common in SU. In terms of renal functional outcome, SU shows superior results in preservation of renal function.
