Introduction: Conventional transurethral procedures have several disadvantages, such as long hospitalization, hematuria, incontinence. Furthermore, transurethral procedure are notorious for risk of sexual dysfunction.
Objective: This study is to report outcomes of prostatic urethral lift (PUL) to assess its efficacy on lower urinary tracts symptoms (LUTS) and sexual functions in Korean men.
Material and Methods: Thirty nine patients, who were high risk for anesthesia or wanting to preserve their sexual function, underwent PUL in 2017. International Prostate Symptom Score (IPSS), maximum urinary flow rate (Qmax), post-void residual urine (PVR), International Index of Erectile Function-5 (IIEF-5), and Male Sexual Health Questionnaire for Ejaculatory Dysfunction (MSHQ-EjD) were obtained preoperatively and throughout 6 months follow-up.
Results: IPSS quality of life (QoL) and total score significantly improved postoperatively (4.4 ± 1.2 VS 3.1 ± 1.4, p = 0.003 / 18.9 ± 5.8 VS 14.6 ± 6.2, p = 0.04). Voiding symptoms and store symptoms improved (11.5 ± 4.8 VS 8.5 ± 4.4, p = 0.005 / 7.4 ± 2.5 VS 6.2 ± 3.2, p = 0.024). Qmax and PVR did not change by PUL (10.4 ± 4.6 VS 11.0 ± 6.0, p = 0.328 / 44.0 ± 64.0 VS 36.0 ± 34.4, p = 0.611). Sexual function was unchanged or improved (IIEF-5 total: 12.6 ± 6.2 VS 15.0 ± 6.4, p = 0.04 / MSHQ-EjD: 6.6 ± 0.7 VS 5.3 ± 1.0, p = 0.203). Most of adverse events (dysuria: 4 (14.8%), hematuria: 2 (14.8%), urinary retention: 2 (7.4%)) resolved within one or two days. Prostatic abscess occurred to a patient after a month of the procedure, however, patient fully recovered through transurethral abscess drainage without implants removal.
Conclusions: Our results suggest that PUL is safe and effective in improving LUTS and QoL. It also preserved sexual functions. PUL offers viable option for men with LUTS who desire to maintain sexual function with little adverse events.
