Objective: In the present study, we reassessed the factors affecting duration to castration resistant prostate cancer from metastatic hormone-sensitive prostate cancer.
Methods: A total of 154 patients who have metastatic lesion at the time of diagnosis of prostate cancer and underwent androgen deprivation therapy (ADT) were retrospectively analyzed. In this retrospective cohort, we analyzed the clinicopathologic factor on affecting duration to castration resistant prostate cancer from metastatic hormone-sensitive prostate cancer using the Kaplan-Meier method and Cox proportional hazard model. Median follow-up duration was 29 months
Results: The median age of patients was 70 years and median initial prostate-specific antigen (PSA) level was 557ng/ml. The Gleason score was 6 or 7 in 17 patients (11.0%), 8 in 55 (35.7%), and 9 or 10 in 82 (53.2%). We assessed regional node metastasis and visceral metastasis at the diagnosis in 102 patients (66.2%) and 18 (11.7%). In multivariate analysis, PSA ≥20 (hazard ratio 2.845 95% confidence interval 1.227-6.597) and biopsy gleason score ≥8 (hazard ratio 1.955 95% confidence interval 1.296-2.951) were independent predictors of resistant castration.
Conclusions: High PSA and high gleason score were independent predictors of castration resistant prostate cancer in metastatic hormone-sensitive prostate cancer
