LUTS/BPH(구연) Oral Session 3 / LUTS/BPH (Ⅰ) (O-026) Crystal B (3F) 11월28일(수) 13:00-14:00 양성전립서비대증 치료법으로 180W XPS GreenLight 레이저기화술과 경요도전립선절제술의 수술 후 6개월간의 안전성 및 효능 비교분석 고려대학교 의과대학 비뇨기과학교실,¹울산대학교 의과대학 비뇨기과학교실 이동현,안순태,박창후¹김종욱,김제종,문두건 Objective: This study aims to compare the safety and efficacy of 180W GreenLight XPS laser prostate vaporization with transurethral resection of the prostate (TURP) in patients with BPH Materials and Methods: Retrospective analysis was conducted on the clinical data of 128 patients who underwent 180W XPS laser prostate vaporization and 116 who underwent TURP between January 2015 and August 2017. The change in voiding parameters and complications at 6 months postoperatively in the XPS group was compared to that in the TURP group Results: The mean ages of the XPS and TURP groups were 67.5 (±7.1), and 69.7 (±7.1) years, respectively, and no statistical significance was seen among the groups across all subcategories of baseline characteristics. The total duration of surgery was significantly shorter in the TURP group at 54.3 min (±29.4) compared to the XPS group at 60.5 min (±45.4) (p<0.04). However, the total length of stay was significantly shorter in the XPS group at 2.4 days (±1.2), compared to the TURP group at 4.3 days (±1.1) (p<0.001). There was no significant difference between the two groups in the postoperative change in Qmax, PVR, and total IPSS. However, the XPS group showed significantly greater improvement in QoL at 1.9 points (±1.4) compared to the TURP group at 1.1 points (±1.2) (p=0.038). After XPS surgery, 10.9% of the patients reported urinary urgency, 3.1% reported dysuria, 1.6% reported urinary incontinence, and hematuria and in 1 patient. There was no reported case of hematuria requiring transfusion, and no acute urinary retention in XPS group. These complications showed no significant difference compared to TURP group. However, in TURP group, there are more Febrile UTI (8.6%) complications than in XPS group (0.8%), significantly (p=0.004) Conclusions: The results of this study demonstrate that 180W XPS is an outstanding laser treatment device that offers a safer treatment method with fewer complications, with treatment as effective as with TURP. keywords : XPS,TURP,BPH
