Pathologic characteristics of MRI negative prostate cancer

Purpose : The non-invasive treatment trends are getting increasing for prostate cancer patients and the negative finding on MRI are component of active surveillance criteria. We aimed to compare pre-operative MRI finding and final histopathologic outcome.
Methods : Of 384 men who underwent radical prostatectomy between 2013 and 2018 without previous hormonal or radiation therapy, 82 patients showed no suspicious cancer lesion on MRI. And also to evaluate the efficient timing of MRI, we divided the patients into two gropus – who taken MRI before biopsy(n=27) and after biopsy(n=55). We compared final histopathologic T stage, Gleason score(GS) and tumor volume, pre-operative PSA, TRUS volume, PSA density.
Results : Baseline demographic data and final histopathologic T stage was lower between MRI negative vs positive patients (≤pT3a : 89% vs 51.7%, p=<0.001) but GS was similar (GS≤7 : 75.6% vs 81.5% , p=0.816). In other categories, there showed no significant difference. In groups divided as MRI before biopsy and MRI after biopsy, pathologic GS showed significant difference(pGS≤6 : 66.7% vs 25.5%, p=0.007).
Conclusion : The present study showed that the PCa patients who have no suspicious lesion on MRI does not have much relation for final histopathologic outcome. But taking MRI before biopsy might be helpful to decide definite treatment.
