Objective: To evaluate the optimal number of ultrasound-MRI fusion target biopsy cores

Materials and Methods: We identified 151 patients who underwent concurrent ultrasound guided systemic prostate biopsy and ultrasound-MRI fusion target biopsy from October, 2016 through July, 2018 at department of urology of PNUH. All patients underwent mpMRI before biopsy. Promising targets based on PI-RADS scores were subject to target biopsy. PCa detection rates were compared in systemic vs. target biopsies and target vs. target biopsies with different core numbers.

Results: Mean of age, PSA level and prostate volume showed no significant differences in systemic vs. target biopsy and target vs. target biopsy with different core numbers. Biopsy cores and positive biopsy cores were lower in target biopsy than systemic biopsy (4.0 vs 12.0; p < 0.001, 1.0 vs 1.8; p = 0.026, respectively). Positive biopsy core rate were higher in target biopsy (23.5 %) than systemic biopsy (14.9 %), but there was no significant differences (p = 0.666). PCa detection rate was lower in 2 cores target biopsy (20.9 %) than in systemic biopsy (43.7 %, p = 0.007). 4, 6 and 8 cores target biopsies had similar PCa detection rates with systemic biopsy. 6 cores target biopsy showed the highest PCa and csPCA detection rate (45.8 %, 37.5 %, respectively), but there was no significant differences compared with systemic biopsy (p = 0.846, 0.819, respectively).

Conclusion: 6 cores target biopsy showed the highest PCa and csPCA detection rates. 6 can be the optimal number of cores in ultrasound-MRI fusion target biopsy.
