Video-assisted minilaparotomy surgery (VAMS) is a hybrid of laparoscopic and open surgical techniques. A VAMS procedure typically is performed through a minilaparotomy, with a laparoscope and internal retractors placed through stab incisions. VAMS does not require pneumoperitoneum or gas insufflation; instead, the abdominal wall is lifted with internal retractors to provide a working space. Visualization of the operative field
during VAMS may be obtained with both direct viewing and the laparoscope. Conventional instrumentation from both open and laparoscopic surgery may be employed in a VAMS procedure; instruments modified specifically for VAMS also may be used. The VAMS approach is particularly well suited for extraction of an intact solid organ through a minimal incision, such as with VAMS living donor nephrectomy.
In this video we introduce a step-by-step guide of living donor nephrectomy as described below.

Step1: Double decker installation & Retroperitoneal space dissection
Step2: Ureter dissection & Kidney mobilization
Step3: Hilar dissection (Ligation of renal vein tributaries)
Step4: Ligation of Ureter, Renal vessels & Extraction of Kidney
Step5: Oversewing of Renal vessel stumps & Wrap-up
