Feasibility of Single-site Retroperitoneal Kidney Surgery using the SP system : A pilot Cadaver study

"Comment: INTRODUCTION: The da Vinci SP (Intuitive Surgical, Sunnyvale, CA) which is a “purpose-designed” single-port robotic platform is emerging as a new surgical approach and has been reported to be applied to various urologic surgeries. However, it’s feasibility for retroperitoneal partial nephrectomy or other renal surgeries is not yet established. In this study we conducted a pilat cadaver study to establish the feasibility of single-site retroperitoneal kidney surgery using the SP system. METHODS and MATERIALS: The cadaver was placed in semi-lateral position. A transverse anterior incision was made 2cm caudal and lateral from the ASIS. GelPOINT advanced access platform (Applied Medical) placed through incision and SP system was docked. RESULTS: The study was done in the following order. (1) Retroperitoneal lateral and posterior dissection of the kidney. (2) Renal artery dissection by posterior approach. (3) Simulation of posterior renal tumor resection. (4) Renorraphy. (5) Ureter dissection. (6) Anterior dissecton of renal vein and artery. (7) Dissection of kidney upper pole. (8) Renal vessel ligation using titanium clip and laparoscopic staplers. (9) Ureter ligation. CONCLUSION: Use of the SP surgical system for retroperitoneal renal surgery such as partial nephrectomy or donor nephrectomy was feasible in this cadaver study. / Video URL: https://youtu.be/f6x3y95v8zU"
